Register at Rushey Green Group Practice

Our list is OPEN to new patients!

We are happy to register any patient that lives within our catchment area.

There are two ways that you can register with the practice. Which ever way you choose you will need to come into the practice with 2 forms of ID and for a New Patient Health Check in order to be registered.

1. Register in Person

To register in person you will need to come in to pick up a copy of our registration form.
Complete the form and return it to us along with 2 forms of ID. A photograph ID (e.g. passport / drivers licence) AND a proof of address (e.g. utility bill / bank statement).

2. Pre-Register Online via EMIS Access - Click Here

This is a new service which enables you to Pre-register online. You must supply all the information the form asks for.
Once you have completed the online form:

  • 1. Bring in a photo proof of ID and proof
    of address to be approved by a receptionist.
  • 2. Sign the practice patient agreement
    (both you and the practice will keep a copy)
  • 3. To complete the registration process you will
    be booked an appointment for a new patient health check

Registering Adults

Anyone aged 16 or over is deemed an adult.

Upon viewing your two forms of ID you will be given a new patient pack which contains plenty of information about the practice and all the services that we provide.

It will take us appx 5 days to input all your data onto our system after which time you are able to book an appointment to see a doctor or nurse when necessary. We will also ask you to book an appointment with our Health Care Assistant for a New Patient Check.

Registering Children

We can only register children if a parent or guardian is registered with the practice or is registering at the same time.

In order to register a child we will need a copy of their birth certificate when you return the registration form.
We will also need as much information as you can give us about the immunisations your child has received, if any.

Temporary registration

If you are in the area for no more than 6 months we are also able to provide you with temporary registration.

The Practice is open: Monday- Friday: 8am-8pm
Saturday: 9am - 11am

Phones open from 8am and transfer to SELDOC at 6.30pm.
